Oppfølging av voksne med behov for langvarig oppfølging og sammensatte tjenester

This grant program is administered by the Norwegian Directorate of Health and delegated to regional state governors (statsforvalterne). The goal is to strengthen services for adults with long-term and complex service needs by establishing comprehensive, simultaneous, and interdisciplinary services from different levels and sectors. The program aims to improve coordination and integration of care for vulnerable adults requiring multiple services across health, social, and community sectors. Municipalities and intermunicipal companies are eligible to apply for funding to develop and implement these coordinated care models. The maximum total funding available for this grant cycle is 122,585,000 Norwegian kroner, which will be distributed among approved projects.
