Pilotprosjekt for samhandling mellom Nav og kommunale helsetjenester

Helsedirektoratet viderefører målet om å prøve ut systematiske samarbeidsmodeller mellom arbeids- og velferdstjenestene (Nav) og kommunehelsetjenesten for å tilby en tidlig, samtidig og koordinert oppfølging av målgruppen. Ved hjelp av samarbeidsmodellen skal pasienter i målgruppen tilbys rask støtte til å bli stående i jobb, til å unngå eller forkorte sykefravær og til arbeidsrettede tiltak eller utdanning for å komme i jobb. This pilot project grant supports municipalities and inter-municipal companies in developing systematic collaboration models between employment and welfare services (Nav) and municipal health services. The goal is to provide early, simultaneous, and coordinated support to help patients stay in work, avoid or shorten sick leave, and access work-related measures or education to obtain employment.
