About This Opportunity

This funding programme is a partnership between Lancashire County Council, Wyre Council, Fylde Council and Community Foundation for Lancashire. The fund's aim is to engage with community groups and the voluntary sector to support people where they are at risk of requiring future care needs. The objective is to complement existing Adult Social Care Services, to offer prevention and support interventions which address the needs of those who, if they do not receive support, may contact or be referred to Adult Social Services. This is a test and learn programme to identify which interventions have the greatest impact and to identify gaps in current services. Applications should demonstrate how the project will offer prevention and early intervention to enable adults with care needs to be independent for longer, addressing issues such as social isolation, mental health support, decluttering, support for carers, benefits advice, falls prevention, and general support to keep people physically and mentally well.
